The male testosterone creams, Andromen and Andromen Forte, are specifically targeted for use in men with declined or lowered serum testosterone levels. Low testosterone in men is associated with declined libido, fatigue, loss of motivation, decreased muscle mass and strength, depression and mood changes.
General Effects of Testosterone in Men
Testosterone controls the development and maintenance of the male sex organs and the male secondary sex characteristics. These include muscle bulk, facial and axillary hair, changes in fat distribution and deepened voice. It also produces systemic anabolic effects which include retention of nitrogen, calcium, sodium, potassium, chloride, and phosphate. This leads to an increase in water retention and bone growth. Testosterone makes the skin more vascular and less fatty.
The testes do not store testosterone. Once produced testosterone is secreted into the blood stream where it is rapidly adhered to by the protein sex-hormone binding globulin (SHBG). Testosterone to which SHBG does not attach is the biologically available testosterone that is free to act on cells throughout the body. This bio-available testosterone is crucial in determining how well testosterone can work in the body. In order to establish an accurate diagnosis for a patient it is essential to measure the free androgen index or FAI.
This is calculated by the total testosterone level in the blood divided by the SHBG level multiplied by 100. Pathology labs will automatically do this calculation and the result will be the FAI reading.
The measurement of total testosterone in the blood is not an accurate representation of how much testosterone is free to act in the body. A measurement of total testosterone, SHBG and FAI, as well as the pituitary hormones: lutenising hormone (LH) and Follicle Stimulating hormone (FSH) should be undertaken by the doctor to comprehensively assess a patients hormonal status.
Generally a FAI reading of 70 or less is a cause for concern and a reading below 50 may be a strong indication that testosterone supplementation is warranted. Other factors such as pre-existing illnesses, physical, hormonal, psychological and mental health must be taken into account by the doctor before routine prescribing of testosterone.
Testosterone Replacement Therapy (TRT) is a management option for testosterone deficiency and the andropause (male menopause). Male menopause is frequently observed as the male ages, but is not as distinct as the female menopause often because there is a bigger age variance and it is less sudden in onset, but can be more severe in its long-term consequences.
TRT can be used in men for all states of testosterone deficiency. Symptoms associated with testosterone deficiency include:
- Impotency
- Less sexual desire
- Fatigue
- Depression
- Loss of enthusiasm
- Osteoporosis
Much publicity has been given to osteoporosis as a condition experienced by women at the time of menopause and beyond.
Men can develop osteoporosis as well as women, although usually onset is in the latter years of life. The testes generally are strong producers of testosterone until the late 40s to early 70s. A decline in testosterone production leads to an increase in bone loss. Once testicular production slows a rapid and marked decline in bone density can be seen.
